I am counting this as an injection scene. Moka Akashiya is at a hospital and a nurse in glasses approaches her. The nurse suddenly turns her hand into this sharp pointy "witchblade" like thing, and sticks Moka in the neck. Some blood is spilled.

Moka falls to the ground unconscious, while the evil nurse gloats.
